Fuzzz PlayGround will always do its best to grant my dog reasonable love, attention, care & exercise. and the premises will be kept safe & clean. However, I understand that Fuzzz Playground cannot guarantee against accident and absent negligence. I am responsible for my dog. Therefore, I agree to be liable for the full cost of any and all treatments, repairs and replacements of any damage or harm my dog may cause to himself, anyone or anything. (if I am asked to pay) My dog is friendly & good with other dogs. So that means never bit a human or another dog, being shy or cautious is very much okay but not aggressive. I agree that if my dog exhibits any aggressive behavior he/she may be separated from the other dogs and asked not to return. All dogs must: 1- Be current on Rabies, Parvo & Bordetella vaccinations. (copy of the most recent shot record is required). 2- Must be old enough to receive all required shots 3- Be spayed or neutered if 6 months of age or older. 4- Be non-aggressive towards other dogs & people. 5- Fuzzz Playground is not your typical kennel (dogs play all day & only go to the kennels at night) so your dog has to able to handle active play & be in general good health. 6- Must use flea & tick prevention products on a monthly basis. 7- Your dog must have a name tag so we can ID them. 8- All dogs must be on the leash when entering and exiting the facility. I agree to allow my dog to participate in daycare - group play & all forms of fun offered by Fuzzz Playground which means interacting and playing with other dogs. I understand that although these activities are supervised and all dogs are friendly, dogs have nails & sharp teeth and anytime dogs play together there is a possibility of injury from accidents, rough play or combative behavior. I also understand that sore paws & runny poop may be an outcome of the first couple of times my dog comes to play; especially if they are not used to such an active environment. We are all about your dog having fun safely so your dog may come home dirty or wet from all the play. but your more than welcome to purchase your dog a bath & we will bath him/her right before you arrive 😊 If your late for pick up & do not answer our calls we will host your dog for a sleep over on your expense. More importantly your dog will be deemed abandoned and Fuzzz Playground will become its legal owner after 7 days of being in our care without any contact or arrangements on your behalf. (you may be unable to retrieve your dog as we will rehome or relinquish the dog to the shelter). Communication is first! Always make sure to communicate with us any recent changes in your dog health or behavior so we are aware for his safety & the safety of the pack. This authorization and release will remain in force for all mine & my dogs’ visits to Fuzzz Playground. I have read, checked & agreed to the waiver, rules & regulations. Therefore, I hereby release Fuzzz playground & its employees from all liability associated in any of their services. I also certify the accuracy of all information given about my dog & agree not to share, publish or disclose any of Fuzzz Playground’s proprietary information or trade secrets.
